Current, signed lease, if making a rental request
Utility Bill (Electric, Water, Gas), if making a utility request
Proof of income for all household members
Most recent bank statement, showing transactions for 30 days

The applicant must live in Forsyth or Dawson County, GA. If the primary residence is not within Forsyth or Dawson County, please call the United Way at 211 (toll free) for an agency that serves your area.
